Transforming Together

Transforming Together (T2) is a multi-year initiative focused on improving how behavioral health services support children, youth, and families. The goal is to help counties better align, integrate, and expand services to create a more effective and connected system of care.

This work brings together county offices of education and community partners to strengthen collaboration across systems. By working together, counties can build coordinated support networks that better meet the needs of students and families.

As statewide efforts like the Children and Youth Behavioral Health Initiative (CYBHI) and the California Community Schools Partnership Program (CCSPP) expand services in schools, Transforming Together helps counties incorporate these changes into a broader, long-term vision for improving behavioral health systems.

 
 
 

Our Goal

When systems are fragmented, professionals often spend more time navigating barriers than meeting the needs of the people they serve.

For example, a school counselor may struggle to find mental health support for a student. A probation officer may not know who else is working with the same young person. A pediatrician may treat a child without knowing about challenges like housing instability.

When systems are better connected, professionals can focus on their areas of expertise. This leads to more efficient coordination and better outcomes for children, youth, and families.
