Transforming Together

Transforming Together (T²) is a multi-year initiative focused on improving how behavioral health services support children, youth, and families. The goal is to help counties better align, integrate, and expand services to create a more effective and connected system of care.

This work brings together county offices of education and community partners to strengthen collaboration across systems. By working together, counties can build coordinated support networks that better meet the needs of students and families.

As statewide efforts like the Children and Youth Behavioral Health Initiative (CYBHI) and the California Community Schools Partnership Program (CCSPP) expand services in schools, Transforming Together helps counties incorporate these changes into a broader, long-term vision for improving behavioral health systems.

Our Goal

When systems are fragmented, professionals often spend more time navigating barriers than meeting the needs of the people they serve.

For example, a school counselor may struggle to find mental health support for a student. A probation officer may not know who else is working with the same young person. A pediatrician may treat a child without knowing about challenges like housing instability.

When systems are better connected, professionals can focus on their areas of expertise. This leads to more efficient coordination and better outcomes for children, youth, and families.

Partners in Community-Led Systems Change

To support this work, the Collaborative Leadership Working Group (CLWG) provides implementation support, tools, and technical assistance to teams in four demonstration counties.

The CLWG includes leaders and experts in community schools, behavioral health, community engagement, and county governance. Members represent state agencies, county offices, service providers, and nonprofit organizations across California.

T²uses input from CLWG members to develop guidance for counties. This guidance helps communities address local challenges and opportunities while improving behavioral health systems for children, youth, and families.

Transforming Together (T²) is a multi-year initiative that supports California counties in aligning, integrating, and expanding services for children, youth, and families. Its purpose is to help county leaders move from disconnected and duplicative programs—efforts that were never designed to work together—to a coordinated and comprehensive system of supports.

In practice, this means building the structures and relationships that turn broad goals into concrete, cross-system change.

Establishing a Countywide Ecosystem of Care

Building a new Ecosystem of Care represents an opportunity for local agencies to transform how they collaborate, share resources and data, coordinate care and services, and partner with families and youth in ways that honor community wisdom and strengthen outcomes. The first step in building a new Ecosystem of Care must include securing broad, high-level county leadership support for this change and keeping leaders engaged and focused on steering the change process.

Using Data and Community Engagement to Assess Needs and Gaps, Identify Priorities and Measure ProgressSetting a shared vision for delivering child and family services differently is just the beginning. For a vision to lead to change, it must be coupled with a clear articulation of priorities and with concrete goals to measure progress. In addition, implementation is more successful when communities being served help co-create these priorities and goals–thus helping to ensure any new system reflects what is most needed.

Universal Array of Services & Workforce Expansion Necessary for an Integrated Children's Ecosystem of CareThis chapter moves from shared vision and goals to implementation by outlining how counties can organize, align, and deliver services and supports across agencies through what we term a universal array of services. The universal array is not a new tool or program; it is a conceptual and structural framework that operationalizes California’s Ecosystem of Care—the collective, cross-agency environment that supports children, youth, and families.
