5141.32 - Health and Safety Hazards
- Parents/guardians will be required to provide a written, provider generated immunization record as proof of their child’s immunization status. This record will be the only acceptable document. Parents/guardians will no longer have the option of signing the California School Immunization Record (CSIR) to verify the immunization dates are valid as listed. This places all public and private California schools on a “No Records, No Registration” policy by law. This restriction applies to new enrollees only; interstate and intrastate transfer students are exempt. (The 10 day grace period for receipt of immunizations currently due has been eliminated for new enrollees effective March 5, 1986.) Pupils age 7 years or older are required to have a total of three doses of any combination of tetanus and diphtheria (Td, DT, DPT). The previous requirement was four doses.
- Parents of new students enrolling on or after March 5, 1986, will no longer be able to provide immunizations dates from sources other than those described above (for example, from baby books, or from memory). In addition, parents must not sign the section of the California School Immunization Record (CSIR) reserved for verification of immunizations if the medical records are not available. This section will be deleted in future versions of the CSIR.
- Parents requiring a provider generated immunization record, but who have difficulty, or limited access to the original provider, may be referred to any physician or Public Health Department service during clinic hours for reconstruction of the immunization history. A California Immunization Record (CIR) can be completed and stamped after reconstructing an immunization history by clinic staff familiar with the recommended schedule of immunizations. Any vaccine doses gathered through this process will be noted by writing “by history” under the Department of Public Health stamp on the card.Some physicians or medical groups may provide parents with computer generated immunization records. These records can be accepted as valid proof of immunization history for school entry as long as the following information is included: student’s name, birth date, each vaccine administered, date of each administration (month, day and year), name of physician or agency.
- The CSIR shall be forwarded to the school nurse for review and monitoring.
- The school nurse shall determine admission status, then calculate the schedule for additional immunizations if necessary.
