5141.11 - Student Emergency Cards

1. Each school or regional office shall keep a current Student Emergency Card for each student enrolled. This card must be signed by the parent or guardian of the student.

2. In addition to authorizing needed emergency care, the Student Emergency Card shall serve to verify that the parent or guardian has received the Education Code sections concerning their rights and responsibilities and a copy of the Discipline Policy.

3. Consent for Treatment of Minor - This form authorizes school and medical personnel to render emergency treatment.
