5141 - Serious Pupil Accident/Illness

The principal is responsible for assuring that the following procedures are initiated when necessary. The principal shall distribute and review with each teacher the following procedure for responding to serious pupil accident/illness.

If serious pupil accident/illness occurs, the teacher or aide under the direction of the teacher, shall do the following:

  1. Obtain emergency papers for pupil respect and carry out any special procedures as stated in these papers by the parents.
    • IF THE SITUATION IS LIFE THREATENING, CALL 911 FIRST AND SAY IT IS A MEDICAL EMERGENCY AND THEN CALL THE PARENT. A SEIZURE THAT LASTS OVER 10 MINUTES IS TO BE TREATED AS AN EMERGENCY SITUATION UNLESS OTHERWISE SPECIFIED ON SPECIALIZED HEALTH CARE PLAN.
  2. Call the nurse.
  3. Immediately attempt to contact parents by telephone in the following order, and document all attempts:
    1. Home
    2. Work
    3. Person to contact on Student Emergency Card
  4. Call the principal to report the incident. This can be done after the immediate situation is under control or by another employee if available. The principal shall immediately come to the site. If the teacher or aide is unable to contact the principal, call in the following order:
    1. Regional Manager
    2. Student Services Director, Student Services Division
    3. Student Services Assistant Superintendent, Student Services Division
  5. If the student must be transported to the hospital, school employee must accompany the pupil.
  6. If the pupil is to be transported to the hospital the accompanying employee shall carry ALL emergency papers with him:
    • Consent for Treatment Form
    • Student Emergency Card
    • Medication Form
  7. The individual who goes to the hospital will report back to the principal as soon as possible to report on the student’s present condition.
  8. If the parents have not been reached before the pupil is taken to the hospital, the principal will continue to make every effort to contact the parents. This may involve requesting assistance from the Sheriff or Police who may authorize transporting the pupil. Document all attempts.
  9. If a situation arises which a teacher feels might turn into an emergency, contact the principal or regional manager.
  10. A report of incident should be prepared the same day encompassing all relevant information, signed, dated, and placed in the student’s cum file with copy to principal (office file) and nurse. In the case of an accident, the Standard Pupil Accident Report form should be filled out and sent to the principal the same day as the incident.

FOR OTHER THAN INCIDENTS DESCRIBED ABOVE ONLY FIRST AID PROCEDURES ARE TO BE FOLLOWED. NO MEDICATION IS TO BE GIVEN INTERNALLY OR APPLIED EXTERNALLY.
