5125.1 - Procedure - Releasing Student Permanent Records

1. When a request for a student’s records is made from another agency or school district, a log will be filled out indicating who requested the records, where they are being sent, when the records were sent (month/day/year), and the initials of the person complying with the request.

2. At the end of each school year, inactive files and records of students who have completed programs may be returned to the student’s home district.
