3515 - Personal Injury
Each employee shall complete and maintain current information on an Emergency Information form (Exhibit 3515) filed in Human Resources as a protection in the event of serious injury. Human Resources shall request employees to update information annually. The employee’s supervisor and/or worksite supervisor may also require the employee to complete a similar emergency information form as further protection.
First Aid materials shall be available at all facilities maintained by Risk Management Services. All materials must be CAL-OSHA approved and kept in adequate supply.
The Office of the County Superintendent of Schools participates in the State Workers’ Compensation program. Every employee who suffers an injury on the job, no matter how slight, must immediately notify his/her immediate supervisor. Risk Management Services must also be contacted in order to help expedite the processing of a workplace injury and to receive medical attention as needed.
