3313 - Equipment Maintenance Agreements - Procedure

Purchase Orders for Maintenance Agreements

The purchase order must include the following:

  1. Description of equipment, including serial and model numbers.
  2. Location of equipment
  3. Term and cost of agreement
  4. Services to be provided under agreement
  5. Name of contact person to notify regarding the service arrangements.

If processing a requisition for an annual equipment maintenance agreement, check the annual box to ensure the requisition is automatically rolled each fiscal year.

All maintenance contracts are routed through Contracts for approval. Send any paperwork referencing your requisition to Contracts.
